If you got your Medical as a student pilot, then you qualify for Basic Med as long as your doctor signs off on the check list. Only you and your doctor ever get a copy of that check list. The only info that goes to the FAA is the date that the checklist was signed and the date you took and passed the online training so they know you are current.
